School age immunisations registration
Dorset Healthcare has partnered with Inhealthcare to provide a fully digital immunisation service for school age children in Dorset. This means you can provide consent for your child to receive their immunisations online at your convenience on any device, anywhere and at any time, using the following simple two-step process:
- Register your child on the Inhealthcare platform
- Complete a short online form.
Please note: you must complete both steps for us to be able to immunise your child.
How to register your child
The simplest way to register your child is by using their NHS number and their date of birth. If you do not know your child’s NHS number you can use their date of birth and full name, as held by their GP.REGISTER HERE
Once you have successfully registered your child you will receive an e-mail confirming this and asking you to complete the online consent form
If you are unable to register your child online please provide your child’s details here and we will register the child for you and send you the link to the online form.
How to complete the online form
Once your child has been registered you will receive an e-mail requesting that you complete a short online form for you to confirm you want your child to be immunised and ask some health questions to make sure they can have the vaccine in school.
You will need the six character session code shown on the offer letter you received either from school or direct from the school age immunisation service to complete this form. This ensures that your child is allocated to the correct session.
If you are unsure about the answer to any of the health questions e.g. about previous vaccinations please check with your GP.
If we have any questions regarding the information you have provided one of our nurses will contact you, please therefore provide a daytime telephone number if possible.
If your child is school educated, the date by which the online form must be completed is shown on the offer letter and is normally three working days before we start immunising at your child’s school. This is to ensure that we order the correct number of vaccines. You can still complete the online form after this date but will be offered an appointment at a catch-up clinic at a community venue instead.
Please note: we are not able to immunise a child unless this form has been completed. You must have parental responsibility for the child to provide consent for them to be immunised.
What if my child has additional needs?
The online consent form includes a question regarding additional needs. If your child has additional needs please tell us and one of our trained nurses will contact you to discuss whether any special arrangements need to be made for your child to have their immunisation.
What if I do not want my child vaccinated?
We will only immunise a child if we have received consent from somebody with parental responsibility.
However if you decide you do not want your child to be immunised, we recommend that you still register your child and complete the online consent form so that we are aware of this when we visit school. Providing a reason will also help us plan and improve the service.
What if my child becomes ill after I complete the form?
If your child has a serious illness after you complete the online consent form please let us know by e-mailing firstname.lastname@example.org or phoning 01425 891162.
Our operating hours are 9am - 5pm, Monday to Friday. You can leave an answerphone message outside of these hours
What if I am unable to use the online service?
If you are unable to complete the process online because you do not have an e-mail address or you do not have internet access, we can register your child and/or complete the form with you over the phone. Please ring us on 01425 891162 between 9am - 5pm Monday to Friday and one of our team will be happy to assist.
